Dental Clinic for USD #248 students

 

   

THESE SERVICES ARE COMPLETELY OPTIONAL. YOUR CHILD WILL ONLY HAVE SERVICES PROVIDED WITH WRITTEN CONSENT.

 

This year's Dental Clinic/Program is scheduled for January 8-12th at RVH and January 15-20th at the Middle School and High School for  USD #248 students. In an effort to comply with Kansas State Statue 72-5201, annual screening inspections, USD #248 is partnering with Community Health Center of Southeast Kansas (CHC of SEK), to provide this service. They will be offering their services to all students, Pre-K thru 12. The services provided include a dental screening, exams, cleanings, x-rays, fluoride varnish and/or dental sealants and even possible fillings for those students who have signs of tooth decay.

    A form was sent home with your child that explains the dental services provided in further detail as well as the dates of the onsite clinic. Included in the three page packet, you will see a consent form that must be filled out in order for your child to participate in any of the services. It must be filled out in full and signed by a parent/guardian in order for your child to receive services. Without parent/guardian consent, your child/children, will be excluded from these services. If you wish to opt out of any specific service or all services, please check the provided boxes on the form in the space provided, sign and return to the school. This shows us that parents have seen the form and are aware the program is available. It is NOT mandatory that your child participate. Unless the form is returned with instructions for your child to be opted out of all services, your child will participate in a visual screening performed to assess for concerns. You will then receive a referral letter. Again, without a signed consent, no treatment will be provided. However, you will be sent a letter letting you know what was visualized and if a referral is recommended. 

    These services are provided by a dental hygienist and/or a licensed dentist. This would be the same as taking your child to a dental office. Therefore, if services are provided to your child (with your consent) CHC of SEK will process the claim thru your insurance making it unnecessary for you to visit a dentist for a follow-up for another 6 months. (Unless you are otherwise notified). Further more, this means your insurance will be billed but there will be NO out-of-pocket expense to the parents/guardians. For those without insurance, there will still be NO out-of-pocket expense to you.
